Community Health Needs Assessment

The Washington County Health Department in partnership with Meritus Health Systems (formerly the Washington County Hospital Association) is conducting a community health needs assessment. Please take a few minutes to visit the site at the link below and fill out a copy of the questionnaire.

The results of the survey will help us to form policies and guide us to providing better services to improve the health of the entire county. All responses are strictly confidential.
