This form is required for the safety of your child. We must have the right medication, in the right bottle, with the doctor's and the parent's permission on a signed PMOF.
How long should my child be out with a fever?
Your child must be fever free at least 12 hours without medication.
Can my child bring cough drops to school?
Check individual school policy.
Can my child carry an inhaler/epi-pen in school?
Yes. A Physician's Medication Order Form must be completed by your Primary Care Provider.
Do head lice jump?
No. Head lice are mainly spread through head to head contact but are also spread through the sharing of combs, brushes, hats, scarves, clothing and even stuffed animals.
My child has special needs and will need extra help at school. What can I do?
Call your School Nurse to set up a meeting to address your child's needs. If necessary, an Individual Health Care Plan will be developed. This information is then shared with your child's teachers, bus driver and administration.
Will you call me every time my child comes to the Health Office?
Not always, but if this is something you feel strongly about, we will do so. We always send home a copy of the Health Office Report with all students. If there is obvious illness or injury, we make every effort to contact someone. Please keep your emergency numbers updated so we can always reach someone. Also, please keep us updated on any health related issues.
Will my child get their hearing and vision checked in school?
Yes, your child will get their vision and hearing checked periodically. The screenings are done in K, 1, 3, 5 and 8th grades. The Lions Club does the initial screening for vision. The Health Department's Vision and Hearing staff will do follow-up vision screening and the initial hearing screening. Parents are notified before the screenings. If the child fails the screening, a letter will be mailed to the home for follow-up. Scoliosis (curvature of the spine) screenings are done in 6th, 7th and 8th grade. These are done by the school nurse. You will be notified by mail if follow-up is needed.
Can my child self-carry/self-administer emergency medications?
Students are allowed to carry emergency medications, i.e. inhalers or Epi-pens, if ordered by their Health Care Provider. Health Office staff can provide information and necessary forms.
What are Student Health/Family Information Forms?
This information is extremely important and is kept on file in the Health Office. Should an emergency occur or the need arise to make contact with the home, we rely on this information. Please notify staff of any changes in telephone numbers.
