DESCRIPTION:
|
The Washington County Health Department Nursing Division is seeking a part-time contractual Community Health Nurse II. Duties include: contact ACCU clients by phone, written correspondence, or face-to-face interview following receipt of referral; conducts initial home based assessment of maternal/infant client to identify risks through data collection and analysis. Forms and implements a plan of care. Provides case management for the client, including counseling, education, case coordination, referral services, planning and implementing interventions, and evaluation. Coordinates and communicates with interagency care. Maintains written record from initial contact through completion of service to ensure complete accurate documentation of patient care. Provides Children with Special Health Care Needs Case Management and Care Coordination services using the nursing process. Provides outreach to local physicians on Services available to Children with Special Health Care Needs. Engages in professional training through in-services, staff meetings, informal discussion and reading of educational materials to maintain professional skills and knowledge. Utilizes nursing process to provide comprehensive community health nursing clinical services to and community settings using a holistic, family centered, multi-disciplinary approach.

MINIMUM QUALIFICATIONS:

EDUCATION:

A Bachelor's Degree in Nursing from an accredited college or university

EXPERIENCE:

None
NOTE: A bachelor's degree in a related field and one year of experience as a registered nurse may be substituted for the required education. Two years of experience as a registered nurse may be substituted for the required education.

LICENSE:
|
Candidates in this classification must possess a current license as a Registered Nurse from the Maryland Board of Nursing.
